If you can print out the form, fill it out, and bring it with on your first visit it will save time here in the office. If you have any testing done by other providers - x-rays, blood work, etc., please bring copies with you.
If you are pregnant, please also complete the pregnancy case history. If the visit is for a child, please complete the child's case history.
